PEBC: What You Need to Know: Fibrates

Examples: Fenofibrate, bezafibrate, gemfibrozil

Indication: elevated triglycerides (and to some extent, reduces LDL cholesterol and increases HDL cholesterol)

Dosing:

Fenofibrate and bezafibrate: once daily

Gemfibrozil: twice daily before meals

Adverse Effects:

-myopathy

-gastrointestinal upset

-abdominal pain

-increased liver function tests

-gallstones

Drug Interactions:

-increased risk of rhabdomyolysis with statins and colchicine

-gemfibrozil is an inhibitor of CYP2C8/2C9, careful with substrates such as sulfonylureas, repaglinide and rosiglitazone
